you for your interest in being involved in the DCCC membership offer for DCB
junior club members. I am pleased to
provide further information along with an application form.
It
will assist in the processing of applications if you take note of the
following:
- An application form
is for one person only – please take as many copies as necessary.
- Ensure that the
application form is signed by a parent / guardian along with a club
official and that a passport sized photograph is also included.
- Please note that individual applications will
not be accepted. All forms must be
returned to
